Diagnosis: How is Arachnoiditis diagnosed
The diagnosis of arachnoiditis is often complex and requires a combination of clinical insight, imaging, and the exclusion of other causes. The patient's symptom pattern is central; imaging is supportive but by no means always shows the full severity of the condition.
1. The role of imaging
An MRI of the lumbosacral spine is the most commonly used examination. The images may show indications such as:
- Nerve root clumping
- Empty sac sign
- Disruption of cerebrospinal fluid (CSF) circulation
- Adhesions or scarring
However, in many cases, MRI findings remain subtle or even normal, while patients may experience severe symptoms. Therefore, a normal MRI does not rule out arachnoiditis.
2. Disrupted CSF flow as an indirect signal
A disruption in the circulation of cerebrospinal fluid (CSF) around the spinal cord can be an indirect indication. This can be accompanied by symptoms such as headache upon changes in pressure, tinnitus, blurred vision, or dizziness. On MRI, this may appear as abnormal fluid distribution or an abnormal shape of the dural canal.
3. Tarlov cysts: possible indication
Tarlov cysts, or perineural cysts, are cerebrospinal fluid-filled sacs attached to the nerve roots. They are frequently seen in people with arachnoiditis, particularly around the sacral region. Although they can also occur in other conditions, they may indicate increased pressure or chronic inflammation of the nerve roots. Consequently, they constitute a possible indication of underlying CSF dysfunction or irritation of the arachnoid membrane.
4. Diagnosis based on the overall picture
The final diagnosis is based on:
- The pattern of symptoms (such as nerve pain, loss of sensation, muscle weakness, bladder and bowel complaints)
- Exclusion of other neurological causes
- MRI or possibly CT myelography
- Supportive neurological tests
In some cases, imaging remains only partially visible, especially in early or milder stages. Therefore, it is important that physicians remain alert to the clinical picture even without clear MRI abnormalities.
Treatment: What can help
Arachnoiditis is a chronic and complex condition for which, unfortunately, there is (yet) no curative treatment. Nevertheless, there are various options to alleviate symptoms, limit damage, and improve quality of life. Treatment almost always requires a combination of medical, physical, and psychological support, tailored to the individual.
Anti-inflammatory and nerve protection
Calming the inflammation is an important goal, especially in the early stages. In the first three months after the onset of arachnoiditis, a short course of Methylprednisolone (a powerful anti-inflammatory) can play an important role in limiting damage to nerve tissue. This can sometimes be used again during flare-ups. In addition, anti-inflammatory supplements, antioxidants, and nerve-protecting agents can provide support. Lifestyle, nutrition, and sufficient rest are also important factors for protecting the nervous system as effectively as possible.
Pain relief
Chronic nerve pain is one of the most severe types of pain. Treatment may consist of neuropathic medication, opiates, TENS therapy, topical creams, or, in severe cases, implants such as a nerve stimulator or intrathecal pump. Often, a combination is necessary, tailored to the type and intensity of the pain.
Movement and daily functioning
Light, regular exercise helps keep the nerves supple and slows down further adhesions. Physiotherapy or exercise therapy can provide support, as can assistive devices such as a cushion, walking stick, or wheelchair, if necessary. The goal is to maintain as much independence as possible, within the limits of the body.
Mental and emotional support
Long-term pain and disability can be a heavy burden. Anxiety, frustration, or depressive feelings are understandable reactions. Psychological counseling, peer support, or relaxation techniques can help increase resilience. Good information and support are also important for loved ones.

Every patient is different. What works for one person does not automatically work for another. Collaboration between healthcare providers, good communication, and recognition of the condition are crucial. Early recognition offers the best chance of stabilization of symptoms.
